Freedom of Thought - Fair Punishment for the Criminally Insane

The criminally insane are incarcerated as dangerous felons and offenders, who, due to an absence of criminal responsibility, cannot be judged. But did they really not know what they were doing?

Are they rebels without a cause? Can there be fair punishment for the criminally insane? How does anyone live with the prospect of having no prospect of freedom?
